Financial Agreement and Policy Statement : Excel Adventist® Academy Missouri City TX

Registration fees are due at time of registration and are non-refundable.
Each month's tuition must be paid no later than the fifth business day of the month
There will be a late fee in the amount of $25.00 if tuition is paid after the fifth business day of the month
There will be a $30.00 charge for returned checks
Returned checks will not be re-deposited
The amount of returned checks and related fees must be paid in cash or by money order
Once a check has been returned, future payments must be made in cash, or money order
A student will not be allowed to attend classes if tuition is not paid by the 15th of the month
Should an account remain delinquent in violation of the financial agreement per the registration, the student(s) will be officially withdrawn from the school
Students with outstanding balances will not be re-admitted unless all previous indebtedness is paid in full
Student's record will not be released until all financial obligations are paid in full
Tuition not paid at the end of the school year will be sent to a collection agency

I agree to the conditions in the Policy Statement above and agree to pay my tuition as indicated: In ten (10) equal payments due the first business day of each month of the school year but not later than the fifth business day In two (2) equal payments due between the first and fifth business day of August and January of the school year respectively (5% discount) In one (1) equal payments due between the first and fifth business day in August of the school year respectively (10% discount)

Enrollment Requirements for New Students

A completed New Student Application.
Birth Certificate, Hospital Statement, Notarized statement, Passport or Visa (to verify birth date)
Immunization verification for all students as required by Texas Law. Students MUST have all vaccinations required for the appropriate age level.
Cumulative records and the student checkout sheet from the previous school attended.
Last report card (original)
Medical Exam
Vision/Hearing Screenings required by Texas state law for all first-time students ages 4 years (by September 1) through 8th grade. Screenings may be given by the student's primary doctor or Pinewood Screening Services on campus visit.

Enrollment Requirements for Returning Students

A completed Application Returning Form.
Updated Immunization Record
Vision/Hearing Screenings required by Texas state law for students who will be 4 years old by September 1 and those who are entering Kindergarten, 1st, 3rd, 5th, or 7th grades. Screenings may be given by the student's primary doctor or Pinewood Screening Services on campus visit.

All applicants files will be reviewed before admission. Full acceptance is not granted until all application documents have been submitted.
